Mission and Vision

 Despite availability of proven effective medical, pharmacologic and non-pharmacologic therapies, many Americans with chronic illness go undetected and untreated due to varying reasons. Poverty, low socio-economic status, access, cost, and quality of care may be contributing factors in not achieving optimal health status for most people. We are in business to bridge the gap in health disparity among underserved individuals and families. Our goal is to provide exceptional, affordable, accessible, highest -quality and evidence-based health care for Americans, especially the underserved in geographically disadvantaged areas with high concentration of chronic illness and drug dependency related deaths. We are committed to help our patients manage chronic illness by providing consultation, educational programs and support and resources to meet those needs. We seek to engage patients who has successfully met health care goals as peer support to motivate other individuals struggling with similar health issues. We are committed to providing clinical rotation sites and preceptorship programs to train the next generation of primary health care providers
